• If possible, please print and complete the forms listed below and bring them to the office along with your insurance card(s) and driver's license.This will expidite the check-in process.  
• If you are unable to print the forms, please arrive early to fill them out in the office. It will make the process easier if you read over the forms, even if you can't print them so you will be prepared to answer the questions listed on the forms in person. Specifically knowing your medication/medical problem lists and related contact information will help expidite the process. 
• Please go to the Insurance tab and review the list of Our Participating Plans to make sure we are in your plan. Feel free to call 314-434-1120 if you have questions about your insurance. We are happy to help.
